Family Planning:Counseling on preconception, birth control methods, pregnancy testing, pap smears. By appointment only.

EPSDT/ Well-Child:Outreach, screenings and assessment for children birth through 20 years of age.

WIC: Women, Infants and Children- Nutritional screening, counseling and food vouchers. Screenings by appointment only. For detailed information visit: Kentucky Cabinet for Health and Family Services.

Immunizations:Pediatric and adult.

Fluoride Varnishing:A "Kids Smile " project designed for prevention of tooth decay in children ages one through five.

Diabetes:Counseling and monitoring on diet, exercise and medication.

Cancer Screening: Pap smears by appointment. Includes instruction of self-breast exam. Mammograms for women 40+.

Cardiovascular Disease:: Cholesterol screening & counseling. Blood pressure screening, monitoring and counseling.

Communicable Disease:Reporting, treatment, referral and counseling.

MCH Nutritionist: Counseling on nutrition and special diet (at doctors request).

Vital Statistics: Provides information on birth and death records after 1911; marriage and divorce records after June 1958. Paternity information available upon request.

Do you receive:

Yearly blood pressure checks?

Cholesterol screenings beginning at age 21 then every 5 years if normal?

Blood sugar checks beginning at age 45 then every 3 years if normal (earlier if at risk)?

Vision and hearing screening yearly after age 60?

Yearly screening mammogram for women beginning at age 40 (earlier if at risk)?

Yearly Pap smear for women?

Tetanus shot (vaccine) every 10 years?

Assessment of health risks?
