Mission
The mission of the Kentucky Homeplace Program is to provide access to medical, social, and environmental services for the citizens of the Commonwealth.
Vision/Goals
The goal of the Kentucky Homeplace Program is to educate Kentuckians to identify risk factors and use preventative measures to become a healthier people with knowledge and skills to access the healthcare and social system.
Objectives
- Provide access to medical, social and environmental services by networking with a service provider at the lowest cost possible.
- Educate individuals to access appropriate health care to decrease uncompensated hospitalization and emergency room visits.
- Network with multi-disciplinary agencies acting as a liaison on behalf of individuals and their families.
- Provide education on primary and preventative care to promote a better quality of life.
- Collect and analyze data to identify health trends, socio-cultural and economic barriers to health care.
- Find solutions to bridge the gap between uninsured and underserved individuals and the health care delivery system.

- Free Health Information
- Reduced or no cost prescription medications through various programs.
- Eye exams and eyeglasses through various programs.
- Hearing aids at reduced rates.
- Reduced fee for dental services, including low cost dentures.
- Sliding fee referrals to access lower-cost doctor visits/certain providers.
- Medicare D information.
Kentucky Homeplace is not limited to health care needs only. We attempt to assist any client with any needs he/she may have through contacts, resources and research to meet individual needs.
